Symptom narrative

On Friday, January 16, 2026, I administered the Proquad vaccine to 3 year old, pt in the left arm, subcutaneously. Prior to administering the vaccine, I informed the parent of a possible fever if given as the child's first dose of MMR. Parent agreed to vaccine. Per ACIP guidelines, Proquad should not be given to children under 4 years old due to possibility of febrile seizures.
