Symptom narrative

Site: Itching at Injection Site-Mild, Site: Redness at Injection Site-Mild, Site: Swelling at Injection Site-Mild, Systemic: Diarrhea-Mild, Systemic: Tachycardia-Mild, Additional Details: Day 1 of vaccine patient reported swelling, redness, itchiness and warmth at injection site the size of a quarter about 4 hours after vaccine. Following morning, pt had diarrhea amd rapid heart rate. Went to urgent care, provider recommended adding Pepcid to the Benadryl I recommended. By evening no more diarrhea or fast heart rate. Pt feels she's getting better, no other care pursued.
