Symptom narrative

no adverse event; Medical Assistant calling with a report of an adverse event regarding VAQTA. Medical Assistant stated that they administered a dose of VAQTA to a 4 month old patient during a routine vaccination appointment out of error. Medical Assist stated: "I w; Information has been received from Business Partner/CRO on 12-Jan-2026. This spontaneous report was received from a other health professional and refers to a patient of unknown age and gender. The patient's medical history was not reported. The patient's concurrent conditions were not reported. Concomitant therapies were not reported. On 12-Jan-2026, the patient started therapy with Hepatitis A Vaccine, Inactivated (lot #Z005852, expiration date: 15-Oct-2026) for an unknown indication. On 12-Jan-2026, they administered a dose of VAQTA to a 4 month old patient during a routine vaccination appointment out of error. The vaccine administrator mistakenly grabbed VAQTA instead of VAXELIS (Wrong vaccine used) No additional adverse event reported.
