2o y/o male reported to clinic for routine AOR specific: Anthrax, Series (1) vaccination. Vaccine: Anthrax; Mfg: Emergent Biosolutions; Lot: 300223A; Exp: Oct 2028. 0.5mL of Anthrax Vaccine was drawn and administered IM route in the patient's R arm. Pt was placed on bench in Medical for observation. Pt began to developing pruritic hives at multiple sites on his body, Pts BP: 136/74; SPo2: 90%, P: 156, R: 12. Administered 50mg of Diphenhydramine PO. O2 administered via Nasal cannula at rate 10 LPM. Repeat vitals: BP: 118/74, P: 64, SPo2: 96-100%. Continuing to monitor, EPI-Pen on standby. Vaccine Exemption provided
