On December 31, 2025 at 2:20PM, child (2 years old and 11 months) arrived with both parents and four other sibling for COVID, Influenza, or both vaccines. First RN ("temporary" nurse started 12/29/2025) received all family vaccines from "temporary" pharmacist. Regular staff were out for the office due to the holidays. Pharmacist gave vaccine to first RN. First RN did not verify vaccine against the package box and was only given vials. All injections were given to all other 6 family members. Then, second RN prepared both COVID and Influenza vaccines for child by adding needle to single dose vaccines. Both vaccines were administered at the same time. COVID given in right thigh and Influenza in left thigh. Nurses do not recall which side of the injections they administered due to the large family numbers. Second RN entered the information on the vaccine administration form and both RNs signed the bottom of the form. New nurse protocol is to initial on the Vaccination Administration Form which injections were given by RN. On January 2, 2026, upon entering the data in Immunization Information System, second RN noticed Vaccine Administration error. COVID Comirnaty is approved for ages 5-11 years only. Second RN telephoned the State vaccine for Children program Immunization Coordinators to report the error. Three immunization coordinators were called and only voicemail messages were left. Second RN then called Pfizer Comirnaty toll free customer support line - they were closed for the holiday too. Then, second RN telephoned mother and explained COVID vaccination given to child is for 5 to 11 years old Mother reported that patient was doing well. Child complained of pain at one of the injection site, unknown which leg. Mother gave Tylenol after injection. States child is playing with like his normal self, denies any abnormal symptoms 2 days after the event. Advised mother to continue monitoring behavior and symptoms and report to clinic as soon as possible. There were NO ADVERSE EVENT due to the vaccination. Child did not seek any treatment due to the vaccination.
