Symptom narrative

I received a vaccination on October 2, 2025. The clinic first stated they had no record, then later claimed I received DTaP, by mistake instead of the Td or Tdap which was requested for the mission trip. No documentation was provided however it has been requested. I developed a painful, raised, hot welt approx. two inches in diameter lasting several days. Treatment: Ice, OTC pain relievers. Outcome: Large local reaction. Other: No State Immunization record found.
