Symptom narrative

On 1/20/26 this 14-day infant was seen in the clinic and the provider ordered for the Beyfortus (RSV vaccine for infants) to be administered. The RN incidentally administered Abrysvo (RSV vaccine for pregnant women). The vaccine was administered IM in the left vastus lateralis. This error was reported within 15 minutes of the event. The family had already left with the infant. We contacted the family by phone and gave instructions on potential adverse symptoms to watch (fever, lethargy, fussiness, poor feeding). The family returned with the infant on 1/21/26 for Provider to re-evaluate. The infant was well appearing, vital signs were all normal, and parents denied any adverse symptoms. The family consented for the Beyfortus vaccine to be administered at this time. Beyfortus was administered IM in the right vastus lateralis.
