Symptom narrative

The morning after vaccination at 6am the client noticed pain/discoloration and redness in the left arm opposite the injection site. No self care measures were taken. On 1/22/26 Client returned to clinic for unrelated reason. Client reported at that time continued pain that has not improved swelling and discoloration opposite the injection site. Denies any other injuries. Increased pain with movement and cannot sleep on left side due pain.. Client encouraged to seek medical care for persistent or worsening symptoms. Client has previous vaccinations as a child, but no documentation in his possession.
