Product preparation error

Symptom narrative

ActHIB was reconstituted with incorrect diluent. It was not reconstituted with the diluent in the HIB box. A Merck sterile diluent was used. This was found out 2 days later. Nurses involved with administration were educated. Mother of child was notified of this error. Pt will be revaccinated.
