THE PATIENT WAS SUPPOSED TO GET THE DTAP AND THE PATIENT ENDED UP GETTING THE KINRIX WITH DTAP AND POLIO IN IT. THE PATIENT WAS GIVEN THE IPOL (POLIO) WITH THE KINRIX VACCINE AS WELL.
