Havrix given at aproximately 1205, within less than 30sec., pt. reported feeling lightheaded and "weird". Patient immediately became diaphoretic with dilated pupils, and within 3 minutes following vaccination, reported that her hearing had become affected, which she later described as though she was "hearing sound underwater". Within the three to five minutes following injection, her head rolled to the side during what appeared to be a 3-5 second syncopal episode, throughout which she remained in a seated position. Patient was pale and continued to be diaphoretic, and at aproximately 5-7 minutes following injection, pulse was 62, sats= 97 dropping to 96, BP= 90/60. Pt. described that she had a second round of feeling same sensation, and temporarily lost hearing again for 1 minute. Vitals taken at 1225 were a repeat of afore mentioned measures, though sats had risen to 98. She was released to partner's care at 1310, and before leaving, her sats were 98, BP= 102/70, pulse= 72.Though she was scheduled to also receive a TDaP vaccine during this appointment, it was never administered.
Patient did not report any illnesses for the 30 days leading to today's date.
Patient reports chronic right hemispheral migraines that she has been working with her physician on.
