a dose of PEDVAX HIB was mixed with sterile diluent prior to administration.; This spontaneous report was received from an Other health professional (medical assistant) and refers to a patient of unknown age and gender. The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 15-Jan-2026, the patient was vaccinated with Haemophilus b Conjugate Vaccine (Meningococcal Protein Conjugate) (LIQUID PEDVAX HIB), Suspension for injection (valid lot #Z004969, expiration date: 07-Nov-2027) 0.5 mL administsred for prophylaxis that was mixed with sterile diluent solution for injection (lot # and expiration date were not reported) prior to administration (Product preparation issue).
