Public Health administrative staff was entering vaccinations into vaccine registry when she was unable to put in Varicella. Upon further review with the nurse, the issue was that an MMR-V was given along with the Varicella. RN talked with other nurse who was doing the clinic and figured out how this happened and went over ways to prevent this in the future. Public Health nurse attempted to call parent on three different occasions. First attempt was 1/21/2026 at 1445- voicemail said mailbox was full. Second attempt was 1/21/2026 at 1455- mailbox was available, voicemail left to call PHN back. Third attempt was 1/22/2026 at 0940- another voicemail was left at that time.
