Symptom narrative

This student received an extra dose of MMR and MenACWY on 1/15/26. The error was noted when our vaccine manager was reconciling billing the next morning and found additional vaccine records from immigration for this student. The records from immigration revealed the additional doses administered on 1/15/26 were not needed. Our vaccine manager contacted the Father of the student using a language interpreter on 1/16/26 to notify them of the additional doses received. The father expressed understanding of the incidents and confirmed the student was not experiencing any adverse effects. This report is being filed as a precaution and in accordance with risk management and safety protocols.
