Symptom narrative

(11:50 am) 10 year old in clinic for immunizations (HPV and HepB). Immed. following injection, pt had syncopal episode lasting approx 30 sec. Client bumped head during fall. Immediate VS: b/p 104/70; HR 61; SpO2 98. No head injury observed. Pt responsive within ~30 sec. EMS notified per protocol. EMS reassess b/p 127/80 HR 97, SpO2 98% PERRL. Family declined EMS care/transport following EMS consult with ED physician. Pt and caregiver exited facility; no obvious sequela and /w recommendation ED physician for follow-up (12:17 pm).
