Symptom narrative

Patient stated feeling dizzy after receiving vaccine. Advised to sit down and was given water. Told to remove hat and loosen winter jacket. Still wasn t feeling well and was complaining of difficulty of breathing. Medical staff assessed him and vitals were 97.1 degree temp O2 98% HR 77 and BP 158/98. Ambulance was called due to patient still not feeling well. Chief complaint mild/moderate shortness of breath. Patient taken to hospital by ambulance.
