Symptom narrative

Physician ordered PCV20 and MA gave Pentacel by mistake. Patient received 3rd Pentacel on 11/14/2025 and his DTaP component was given too early. Office vaccination policy was not followed which led to this error. Steps have been taken so this never occurs again.
