Symptom narrative

Pt was given Afluria PF 0.5ml IM on 1/21/26. On 1/23/26, pt was at the pharmacy and informed me that she developed HIVE and had to get an Epipen shot at an urgent care. Pt asked for the ingredient of the vaccine and I provided the patient with the drug insert printed from the manufacturer, Seqirus, website. Also printed State Immunization record to show patient past history of vaccine. However past vaccines were not given at our pharmacy, so no further info on those vaccines were provided. Pt seems alert and well, and was fine after being given Epipen.
