Symptom narrative

Pediatric hepatitis B (Engerix-B birth through 19 years of age) was given to 76 years old by mistake. Technician grabbed and entered wrong Engerix-B shot , system failed to notify wrong age and pharmacist overlooked. Patient was given correct adult Engerix-B on 1/22/2026. He did have any complaint any signs/symptoms of adverse effect of children Engerix-B.
