Reported Symptoms: 10037087:PRURITIS; 10037844:RASH; Narrative: patient was not seen but did call in regards to localized allergic reaction to vaccination. Patient also used OTC products listed previously for symptom control. Other Relevant HD: Patient received vaccine on 10/28/2025. Patient called 11/4/2025 and per note: Reports getting an injection (RSV) in left deltoid and his entire left arm broke out in hives. States the itching is keeping him awake at night. They are getting some better but still itching a lot. He also started new b/p med, Lisinopril started on 10/28. Hives is only on left arm. He has been using Benadryl cream with temp relief. and Calamine lotion. Patient contracted by RN 11/5/2025 and per RN note: Spoke with patient and reviewed the above note, discussed this is an allergic reaction. Patient has been using Benadryl cream but hasn't taking anything orally to assist with symptoms. Advised symptoms will improve, need to treat symptoms while body reaction calms down. Recommend to take an oral antihistamine such as Zyrtec daily, can take BID until symptoms subside, can also use cool packs to help decrease itching. Informed I will update allergy list and request if he has further concerns or questions to call back, patient agreeable. Nothing else follows. Considered localized to injection site allergic reaction Patient was taking for symptom control, cetirizine, and using Benadryl cream and calamine lotion, along with cool packs to alleviate the itching.
