Symptom narrative

Patient was due for 1 year old vaccines. Provider orders were MMR and Varicella. Pulled vaccines, verify with license, staff and vaccines were administered. When I returned to document, I noticed that I had pulled Proquad and Varicella instead of MMR. Provider was notified, spoke to parent to monitor for fever. Medication for fever was sent to pharmacy.
