Symptom narrative

Patient noticed a red, swollen, tender, around 2 inch in diameter in size welt around 2 days after receiving her vaccinations. She presented to the pharmacy where she received these vaccines to ask about the reaction on day 12. She stated she had been alternating heat/cold therapy and reaction seemed to be improving in her opinion, still tender to her, and she feels some lumps under the area of inflammation. She will continue to monitor and seek attention if it does not continue to improve.
