Symptom narrative

Adjuvant misuse; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of wrong technique in product usage process in a 74-year-old male patient who received Herpes zoster (Shingrix) (batch number 94J5E, expiry date 02-MAY-2027) for prophylaxis. On 09-OCT-2025, the patient received the 2nd dose of Shingrix. On an unknown date, an unknown time after receiving Shingrix, the patient experienced wrong technique in product usage process (Verbatim: Adjuvant misuse). The outcome of the wrong technique in product usage process was not applicable. Additional Information: GSK Receipt Date: 09-OCT-2025 A pharmacist called to report that only the powder portion and that the diluent was missing. He knew it was important to use the approved diluent and wanted to know if there was a way to get only the diluent portion of the vaccine. During the explanation he mentioned that the diluent for this shot was misused during the vaccination of another patient. The batch number was not provided and a request for this information has been made.
