Patient was given Hep B vaccine instead of HPV vaccine. The ordered vaccine was Hep B vaccine but HPV was supposed to be given per verbal agreement with patient and provider. The incorrect vaccine was ordered prior to administration. Dr called patient via phone at 4:20pm on 1/23/2026 and spoke to patient for about 4.5 minutes. Informed patient that she received the Hepatitis B vaccine and not the HPV vaccine. Informed patient of the following: She was given the hepatitis B vaccine instead of the HPV vaccine. Hepatitis B vaccine is to prevent hepatitis B, which is a virus that can cause liver inflammation. Her receiving this vaccine would not cause harm even if she has had hepatitis B infection before. It also wouldn't cause harm if she already had the hepatitis B vaccine. It just would not help her if she already has immunity from either a previous vaccine or a previous hepatitis B infection. Her labs on Monday will actually say if she is immune to Hepatitis B and if she would benefit from completing the series. I explained that this vaccine is routinely given to healthcare workers, pediatric patients and school volunteers upon patient requests.
