This RN prepared Tdap vaccine, went in to exam room where pt was with sister - who had also been seen for annual physical, mother, and younger sibling. The sister was not recieving any vaccines. I confirmed with mom which vaccine and had Patient confirm his full name and DOB. Once provided we picked which arm (right deltoid) and I admistered vaccine. Patient and his family thanked me, and began to gather their personal items in the room. This RN exited the room and returned to her desk. I then went to the providers office towards that back of the building, the family had still not checked out at this time. When this RN was going back to her desk, I had to walk through the hallway with all the exam rooms and check out window. As this RN walked down the hallway the patient was laying on the ground in front of the checkout window. Another RN caught patient as he began to fall and lowered him to the ground slowly. His mother and other siblings were next to him. As this RN began to help, an adminstrative employee, medical asistant, and Dr (name withheld) MD (listed above) were gathered around the patient. The patient was consious, he looked pale in apperance, and was mumbling. We proceeded to put an ice pack on the patient head, get a cup of water and juice, and take his oxygen saturation. The oxygen saturation was greater than 95%. Another RN completed a neurologically assessment, the pt could correctly identify orientation, setting, person, time and reasoning. We slowly sat up the patient from laying down to have his legs bent for a few minutes then sat him up completely. He sat criss-cross for approximately five minutes. He stated he was okay and proceeded to drink the water, with the ice pack on the back of his neck. He slowly began to regain color in his face. He stated his head hurt and we encouraged him to cointinue sitting down. Aprroximately another five minutes passed and he wanted to stand, another RN helped him stand. He proceeded to go int he bathroom with this RN and his mother to put cool water on his face. Pt was walking equal strides with no difficulty. Another RN walked pt and family out to their car safely. His mother called the next day to update office that pt was acting himself and she had no concerns.
