Symptom narrative

3/12/2025- HPV given 3/13/2025 went to urgent care with abd pain , abd xray neg sent home - but mom stated in that visit about the 4 vaccines given 3/21/2025 went to urgent care headache, generalized pain knees and ankles more pain ,generalized abd pain , nosebleeds ? was ordered some labs but unable to get blood, was prescribed augmentin for possible cat scratch syndrome and left throat pain 4/4/2025 folow up visit - completed augmentin feeling better but remained with throat pain but was more midline 8/12/25 insect bite from stepping on a wasp - given prednisone 10/31/2025- seen in urgent care hives and nosebleeds , cough, congestion 11/12/2025- saw ENT for nosebleeds, nose cauterized 12/17/2025 saw ENT follow up for nosebleeds

Current illness

sleep disturbance, vomiting that had started 24 hours prior to visit

Medical history

Dental caries, pediatric overweight, sprain of anterior ligament of right ankle , social anxiety disorder

Other medications

saline nasal spray, lanolin , ondansetron was prescribed at this visit
