Symptom narrative

Patient was given a 3rd dose of varicella. Patient had already received 2 dose series in the past. He was given a 3rd dose during our visit, because he had multiple state database profiles causing it to appear like he was needing his 2nd dose. When in reality, his 2nd dose had been already administered but documented on a separate profile that originally wasn't seen. Patient having no symptoms. Doing well.
