Symptom narrative

Patient received Dtap vaccine only on 1/27/26 at 1:19pm. At 1:29pm mother notified nurse that patient had started to develop a rash on face, forehead, under eyes. Pt was put back in room RR 31, pulse ox 99%, no wheezing on auscultation. No swelling to eye or mouth. Breathing comfortably. While in room developed a red patchy rash to both upper arms also. 911 called and pt transferred to ER. In ER diagnosed with hives & cetirizine given in ER.
