Administered January 15, 2026 after being stored in the refrigerator for an unknown duration of time at an unknown temperature. No adverse effe; This spontaneous report was received from a Nurse (nurse prescriber) and refers to a(n) 24-year-old patient of unknown gender. The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 15-Jan-2026, the patient was inadvertently vaccinated with improperly stored dose of Varicella Virus Vaccine Live (Oka-Merck) (VARIVAX), formulation: injection, lot #Z012120, expiration date: 23-Jul-2027, 0.5 mL (.5ML) (route of administration and anatomical location were not provided) for prophylaxis. The vaccine was reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT), (lot # and expiration date were not reported).
