A 76-year-old female patient received BNT162b2 omicron (lp.8.1) (COMIRNATY (2025-2026 FORMULA)), on 10Sep2025 as dose 1, 0.3 ml single (Lot number: NAD589, Expiration Date: 23Jun2026) at the age of 76 years, in left deltoid for covid-19 immunisation. The patient's relevant medical history included: "Knee pain" (ongoing). The patient took concomitant medications. Vaccination history included: Bnt162b2 (DOSE NUMBER UNKNOWN, SINGLE), for Covid-19 Immunization. The following information was reported: MUSCLE SPASMS (non-serious) with onset 10Sep2025, outcome "recovered" (11Sep2025), described as "got really bad cramps in her legs and lower body"; VACCINATION SITE RASH (non-serious) with onset 10Sep2025, outcome "recovered" (15Oct2025), described as "rash all around the injection site"; JOINT SWELLING (non-serious) with onset 11Sep2025, outcome "recovered" (11Sep2025), described as "both wrists started to swell up"; GAIT DISTURBANCE (non-serious) with onset 11Sep2025, outcome "not recovered", described as "could hardly step because her knees felt so bad"; ARTHRITIS (non-serious) with onset 11Sep2025, outcome "not recovered", described as "must have caused any arthritis in her knees to flare up"; VACCINATION SITE SWELLING (non-serious) with onset 11Sep2025, outcome "recovered" (15Oct2025), described as "started to swell right around where the rash was around the injection site"; NECK PAIN (non-serious) with onset 15Sep2025, outcome "recovering", described as "back of her neck ached"; PAIN IN EXTREMITY (non-serious) with onset Sep2025, outcome "recovering", described as "her arms ached"; HAEMATOMA (non-serious) with onset Jan2026, outcome "unknown", described as "hematoma on her right shoulder"; SYNCOPE (medically significant), outcome "unknown", described as "fainted". The events "fainted", "rash all around the injection site", "started to swell right around where the rash was around the injection site", "got really bad cramps in her legs and lower body", "both wrists started to swell up", "must have caused any arthritis in her knees to flare up", "could hardly step because her knees felt so bad", "her arms ached", "back of her neck ached" and "hematoma on her right shoulder" required physician office visit. Therapeutic measures were taken as a result of joint swelling, arthritis. Clinical course: Caller is a consumer that reported that on 10Sep2025, she got a Pfizer booster at pharmacy that was prescribed by her doctor. She is 77 and she has gotten several of the shots. She got the first shot and has gotten several boosters after that and never had a bad reaction. She was shocked she could not get it at the doctors office and was told that they are giving them at the pharmacies now. Caller clarified the product to be the Pfizer Covid-19 Vaccine. Pretty soon after getting the vaccine she got a rash all around the injection site and then it started to swell right around where the rash was around the injection site. That evening she got really bad cramps in her legs and lower body and then her both wrists started to swell up. She went to bed, but said that she did put ice on her wrists. Her knees were never really good from playing tennis when she was younger. She said that after the shot it must have caused any arthritis in her knees to flare up. She thought it would get better with all of that crazy stuff. Slowly it seemed to get worse and she could hardly step because her knees felt so bad, her arms ached, and the back of her neck ached. It has not really gotten much better. This is like 4 months in now. She went to see a orthopedic surgeon and he looked at her knees. She went to see her primary care doctor on 13Sep2025, to tell him about the side effects and he said that maybe they did not give her the shot right. He did not know, but was speculating. All he did was prescribe it. Medical conditions: None, she is very healthy and only takes vitamins. Since she had the bad knee pain she took Ibuprofen 400mg every once in while, but does not like to take it because it is very hard on the stomach. Rash: Went away about a month later, and the swelling went away at the injection went away about a month latter too. Got really bad cramps in her legs and lower body: Next morning the cramps in her lower legs were gone, but her wrists were swollen up. The cramps resolved by the next day 11Sep2025. Both wrists started to swell up: Noticed the swelling the next day on 11Sep2025. She put ice on her worsts and the swelling went down down later in the day on the 11Sep2025. Knee pain: It got worse at first and it is starting to get somewhat better, but is still worse than it was before she got the covid shot. Could hardly step because her knees felt so bad: It started on the 11Sep2025, but continued to get worse. Her arms ached: Started pretty soon after she got the shot, maybe in mid Sep2025, close to when she got the shot. Her arms were very sore and her knees and neck. Her arms are lot better now. Back of her neck ached: Stated that it occurred the same time as her arms aching. It is still ongoing, but has gotten much better. Indication Continued: In the past she never really had any bad reactions and her doctor and her thought it would be a good idea to get it. Stated that she has something else happened also and is not sure of the exact date, the doctor has the date. Her knees were still hurting and she was sitting down to her breakfast and all of a sudden her right shoulder was really sore and the next thing she knew she was on the floor. She thinks she saw the doctor on 13Jan2026 and reported that she had a hematoma on her right shoulder. She does not know if it is related to the shot, but she has had so many issues that have happened that she did not have before that makes her think it is related to the vaccine. Never in her life has she fainted.
