Dtap/IPV/Hib (pentacel) was ordered Vaccine for patient for well child visit. student was preparing and administering vaccines for patient. Supervising LPN did not even comprehend that student had grabbed sterile diluent instead of the DTAP/IPV diluent component of the vaccine. So HIB was diluted with Sterile Diluent instead. This error was noticed and caught later in the afternoon when another nurse noticed the extra Dtap/ipv diluent in the box still in fridge. was called and consulted. Hib would be invalid dose and not even activated from the sterile water component. Pt mother was called and notified of the error, and she will be bringing patient back in, in 4 weeks at minimum to receive the correct vaccine. Vaccine that was entered in today was deleted with an appropriate note as to why it was deleted.
