Symptom narrative

Pt states the day after the vaccines she was chilling, shaking, nauseous, and had a large warm, raised area on her arm.

Medical history

hypothyroidism, diabetes, sarcoidosis, anxiety, depression, ptsd, cataracts, arthritis, history of severe abcess 2 years ago, history of shoulder replacement (left side).

Other medications

b complex, vitamin d3, ozempic, irbesartan, wellbutrin, baclofen, levothyroxine, albuterol, metformin, atorvastatin, farxiga, buspirone, trazodone, hydrochlorothiazide

Allergies

morphine, moxifloxacin, gabapentin, corticosteroids

Lab data

pt went to an urgent care facility 1/29/26. was given naproxen for pain/inflammation. pt is to use ice on the area. pt is to monitor for streaking or signs of infection. pt has follow up appointment with pcp soon, but knows to be seen earlier if streaking, signs of infection, etc.
