Symptom narrative

About 30 seconds after vaccine administration patient reported metallic taste in mouth, followed by dysphagia, then loss of vision and syncope. Pt was seated and ice pack was applied to back of neck. Pt regained vision and full consciousness after approximately 10 minutes.
