Administration of only the diluent component of Penmenvy; Administration of only the diluent component of Penmenvy; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate preparation of medication in a 16-year-old female patient who received Men ABCWY NVS (Penmenvy) (batch number 5S723, expiry date 08-OCT-2026) for prophylaxis. On 19-JAN-2026, the patient received Penmenvy. On 19-JAN-2026, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: Administration of only the diluent component of Penmenvy) and inappropriate dose of vaccine administered (Verbatim: Administration of only the diluent component of Penmenvy). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. The reporter believed this had been the first dose of the MenB component and that it should have been the second dose of the ACYW component for patient, but the reporter was not able to verify this at that moment.
