Indication Shortly after getting injections, mother was standing at the doorway and said that her son was starting to get a rash on his face. Writer looked at child's face and noted swelling and rash on right side of nose, right eye and around right cheek with some swelling and rash to left side. Writer went and get provider to come look and grabbed Dr. as well. Dr and provider both examined patient and Dr. requested medication for allergic reaction, no liquid benadryl in office but did have some in pharmacy, order placed. Patient's mother was instructed after assessment by providers to get medication from staff and stay for atleast 30 minutes to be monitored. Mother was agreeable. Medication ordered Dr and PA checked patient's weight and gave verbal order to pharmacy. Order: 4 mg Decadron tablet obtained from pharmacy, and 9 mg liquid benadryl in a syringe. PA administered 9 mg liquid benadryl to patient and writer crushed the four 1 mg decadron tablets and placed in medicine cup with yogurt patient. Prior to giving medication allergies were verified as yogurt was almod flavored, mother let writer and PA know patient drinks almond milk. Writer gave medication via spoon and had child drink bottle in between bites to get medication down. Decadron was given at 4:22pm. Post-Treatment MA placed O2 reader on patient left hand said pules was 121 o2 sat 99%. next spot check was 144 heart rate on right hand o2 sat 97%. Third spot check was on left hand and was 123 pulse 98% o2, patient had no nasal flaring, grunting, issues breathing. Staff member remained with patient to keep an eye on patient. Patient's swelling and rash was noted to be better and breathing remained easy and non labored. Dr. came in spoke with mother let her know if have any more issues once get home to go to ED. Mother voiced understanding. Mother and patient were then taken to front by staff and then taken over to pharmacy so mother could sign for the two medicaitons.. Medication Administered Decadron 4mg tablets crushed. Benadryl 9mg oral suspension via syringe.
