Symptom narrative

Pt appeared to have no adverse reaction. Pt requested flu shot and thought she didn't receive it this year. However, after vaccine was administered upon printing info for pt from profile, found that Pt received this vaccine on 9/26/2025. This was an additional dose off of schedule and pt had already left before I noticed and could notify her.
