Symptom narrative

Patient was seen by Dr on 02/02/2026 for a 7 yr well child check. vaccines were not available on database. mom brought vaccine record with her a copy was given to medical assistant. vaccines were added to the patient vaccine record 2 vaccines were mistaking added wrong. DTAP was added but not DTAP/IPV and MMR was added not MMRV. so Dr. ordered an extra varicella and poli vaccine. this report is being made for 2 additional vaccines given. the mistake was reported by Dr. this incident was also reported to the clinic manger. No adverse events were reported by mom.
