Vaccine coordinator completing vaccine count prior to ordering on 1/27/2026. Noticed Covid Moderna Spikevax 6month-11 year vaccine pulled from refrigerator to thaw on 10/16/2025. Vaccine expires 60 days post thawing which would be on 12/15/2025. Both dates labeled on front of box for staff to reference. Covid vaccine given to this particular patient on 12/17/2025, determined by running report through Immunization Registry System. Covid Moderna contacted, who advised following CDC advisement which is re-vaccination of the covid vaccine. Clinical administrator will be reaching out to family to educated on need for re-vaccination. Program notified, VEARS report being completed, contacting agency to have dose from 12/17/2025 be labeled as "non-valid".
