Product preparation issue

Symptom narrative

The patient presented to the clinic for administration of the MCV4P vaccine but was inadvertently administered a diluent instead. This error was identified during a routine inventory audit. A review of inventory records indicated that this patient was the most recent individual to receive an injection prior to the discrepancy being noted. The patient was promptly contacted, informed of the issue, and advised to schedule an appointment to receive the correct vaccine. Follow-up is currently being conducted to ensure vaccine readministration, pending patient compliance.
