Administered to an Employee, Vaccine Administered Without Temperature Monitoring for 3 Days; Initial report received on 18-Dec-2025. A reporter in the reported that a patient (age and gender not available) received HEPLISAV-B (hepatitis B vaccine) for hepatitis B immunization that had been stored in a refrigerator with no temperature monitoring for 3 days. No medical history or concomitant medications were reported. On 16-Dec-2025, the patient received a dose of a hepatitis B vaccine [HEPLISAV-B (lot number 946907; expiration date, NDC number, and site not available)]. The HEPLISAV-B administered had been stored in a refrigerator with no temperature monitoring for 3 days. No other information was available.
