On an unknown date, the patient received Penmenvy. On an unknown date, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: only Men B component was given on 3 to 4 occasions), inappropriate dose of vaccine administered (Verbatim: only Men B component was given on 3 to 4 occasions), look alike packaging (Verbatim: Flaps on inside of the carton block the view of the vial Packaging is similar to Rotarix) and pharmaceutical product complaint (Verbatim: lot on box different than vial and syringe).
