C/o broke out in a itchy rash 3-4 days ago (Saturday 1/24/26) on body and limbs - as well as two bumps inside mouth. States has hx of eczema. States got vaccine for chicken pox (varicella #1) on 1/9/26. Also c/o swollen lymph nodes, sore throat, HA, and chills at night. pruritic, vesicular rash, lymphadenopathy, fatigue, ST and subjective fever. The patient states that Saturday January 24th he developed a pruritic macular rash that developed small vesicles over the next 12-24 hours. Rash started on the chest and back, by the end of the day he had lesions on his forehead and scalp. A few scattered lesions to the limbs erupted the following day. Has new lesions erupting still, including in his mouth; the ones that started on the 24th are starting to scab. Notes ST, cervical lymphadenopathy, ST, fatigue and subjective fever and chills starting that day as well. Developed a HA yesterday which resolved with APAP. No subjective fever or chills since last night. Denies upper or lower respiratory sx, abd pain, CP, change in coordination or vision. He has not traveled internationally. Denies sick contacts. Has not attended theme parks or an event with a large number of people in attendance. Rough time line, working backwards 21 days: January 24th: Day 0/rash and sx onset. January 23rd: Patient thinks he may have had a few "pink spots" to his chest (isn't 100% sure) but had no other sx, states that he felt perfectly well. Went to a party that night held in his dorm room, estimates 16-20 people were there. January 21-22: Potential start of contagious period. January 13th. Arrived in city, drove up from city with his parents that day. Student is staying at Dorms January 9th: 1st dose of VZV vaccine. January 4th: Attended a small venue concert at Community Space (estimates 40 people attended). January 3rd: Earliest possible exposure. - Notes that all other activities were typical, day-to-day occurrences.

PCR swab of Vesicle VZV test positive 01/27/2026 resulted 01/28/2026. Positive for VZV. Same swab submitted to state lab where genotyping was performed. Vaccine strain of VZV detected, not wild type. Resulted 02/02/2026
