Symptom narrative

Patient got First Dose in August and got Second Dose Today; Initial report received on 30-Dec-2025. A pharmacist reported that a 22-year-old male received the first dose of HEPLISAV-B (hepatitis B vaccine) for hepatitis B immunization in Aug-2025 and received the second dose on 30-Dec-2025. No medical history or concomitant medications were reported. In Aug-2025, the patient received dose 1 of a hepatitis B vaccine [HEPLISAV-B (lot number, expiration date, NDC number, route, and site not available)]. On 30-Dec-2025, the patient received dose 2 of a hepatitis B vaccine [HEPLISAV-B (lot number 947353, expiration date 25-Mar-2028, and NDC number 43528-003-05)] intramuscularly in the left arm. No adverse effects reported. The patient received this dose at a different facility from the first. No other information was available.
