25 yo with no chronic medical hx referred for possible vaccine reaction. Pt received anthrax (Lot #300220A) and yellow fever (Lot # UK218AA) on November 17. He reports that within 30-45 minutes of receiving both vaccines, he developed lip swelling, facial swelling, chest tightness, hives, pruritus, flushing, difficulty swallowing and reported to ER for evaluation. On review of that encounter, provider note states ?On presentation the patient had a full body rash on the face, torso, arms, back and abdomen. Patient had some notable swelling of his lips. Swelling in his ears. Patient was able to talk in full sentences." Due to suspicion of vaccine reaction, EpiPen autoinjector was administer IM and he was also given IV solumedrol, Benadryl, famotidine. Within an hour pt reports improvement in all his symptoms. He was discharged after a period of observation. He had no recurrence of symptoms. Pt was evaluated, 4 days later recounting the same history and was referred to this clinic to determine need for any future anthrax or yellow fever vaccines. This was the first yellow fever vaccine he has received. This was the 4th of the regular 5 dose series of anthrax vaccine that he has received with potential for future booster dose requirements as well. He denies any hx of other allergies. No hx of latex, egg, gelatin, chicken allergies. No prior vaccine reactions.
