Symptom narrative

Vaccine coordinator completing a vaccine count prior to ordering on 01/27/2026. Noticed Covid Moderna Spikevax 6month-11year vaccine pulled from refrigerator to thaw on 10/16/2025. Vaccine expires 60 days post thawing which would be on 12/15/2025. Both dates labeled on front of box for staff to reference. Covid vaccine given to this particular patient on 01/07/2026, determined by running a report through State Immunization Registry System. Vaccine immediately pulled from refrigerator to prevent further use. Covid Moderna contacted due to expired vaccine given, who advised following CDC advisement which is re-vaccination of the covid vaccine. Clinical administrator will be reaching out to family to educate on need for re-vaccination. Vaccines For Children Program notified, VARES report being completed, contacting State Immunization Registry System to have dose from 01/07/2026 be labeled as "non-valid".
