Provider accidentally ordered ProQuad for nursing staff to give. Patient had already received their second dose of ProQuad in June 2025. Nursing staff gave this vaccine. Provider then spoke wiht guardian regarding the extra dose. Patient has a history of localized reaction after receiving this vaccine. Later that evening, there was erythema and swelling at the injection site. Guardian was giving Benadryl. Provider followed up over the weekend and ended up prescribing oral steroids BID x 5 days d/t increase in swelling and erythema. Since then there has been no worsening of sx.
