Symptom narrative

Patient had record of 2 Varicella doses given too close together in December 2025 (should be 28 days apart to be acceptable). Antibody titer was negative so needed 2nd dose. Given 2nd dose with no issue, but vaccine had just been received from Merck cold and in shipping cooler. There was an ice weather event that week. Vaccine paperwork receipt said that vaccine, even though shipped in special cooler, should have been received by 1/31/26. It was received 2/3/26. Even though container was very cold and per CDC Varicella vaccine can be refrigerated for 72 hours, the manufacturer was phoned and they stated they had shipped it the Tuesday before. So while it might be good, we can be 100% sure. We will not count the dose and either titer again or give another dose.

None, none needed. The patient never had any adverse reaction. Told they may need to come back and get vaccination again or titer to see if vaccination is effective after 30 days.
