Symptom narrative

The patient had an appointment on 2/2/26 at 0940. She received 2 vaccines in the office and tolerated them well. The patient then developed a rash on her hands, face and torso around 1600. Her mom gave her Benadryl and took her to the emergency room. Overnight she had a temperature of 100 and the rash was waxing and waning.
