The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 12-JAN-2026, the patient was vaccinated with an improperly stored dose of with Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) injection, lot #Z014735 [which has been verified to be valid lot number for with Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live], expiration date reported and upon internal validation establish as 18-MAR-2027, 0.5 mL administered for prophylaxis. The vaccine was reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT), (lot # and expiration date were not reported) (Product storage error). No adverse effects were reported.
