The pharmacist called seeking a replacement vial of Shingrix diluent stating that he had a box that contained a broken vial. As the pharmacist was preparing the last dose in the multipack box, he did not realize it was broken, so when he gripped the vial to pop the tab off it literally had shards of glass in the skin and his hand and it went immediately in the trash. The batch number was not provided and a request for this information has been made.
